The Plague Doctor is a Unique Lich boss in Outward, found at the bottom of the Dark Ziggurat in the Hallowed Marsh region, and also found in an Unknown Arena in Berg.
Description
The Plague Doctor is an immortal Lich associated with the Decay element, who sustains himself by channeling the Corruption in the Marsh to himself through a complex pipe system.
Once defeated, the player is awarded with:
- Jade-Lich Set
- Jade-Lich Staff
- Ability to trade Tsar Stones for Tsar Weaponry

Combat & Tactics
When you descend to room with the Plague Doctor, the fight immediately starts, so make any preparations before entering.
He mostly uses Decay magic, and is highly resistant to Decay damage. He has very high movement speed, health, and stability. Bringing decay resistance equipment and potions is recommended. The Plague Doctor has a mild vulnerability to Lightning Damage.
The Plague Doctor is not affected by Bleeding or Poison effects.
Skills
The Plague Doctor has knowledge of several Decay skills:
- Launches a volley of tracking projectiles which deal 8 and 17 Damage.
- Leaves mines which deal 10 and 30 Damage. He can detonate them all at once, or have them gravitate around him.
- Summons claws from the ground which deal 10 and 20 Damage.
