Acquired From

Thrice-Wrought Halberd is located in the Cabal of Wind Temple. Defeating all Shell Horrors in the Temple will open a chamber below the entrance (players on the Definitve Edition need to defeat the Vile Illuminator too), which was originally cut off by a barrier. The Thrice-Wrought Halberd will be found in this newly opened area, sticking into the ground next to an Ornate Chest.

A trick to killing all the Shell Horrors is by leading them to the middle chamber, trapping them by closing the doors, and then flipping a switch on the second floor which triggers a blast mechanism.
