Vengeful Ouroboros is a Quest in Outward. It is twelfth in the series of main quests, and is the fourth and final quest of the Sirocco quest-line.
Quest Summary
The Magma level in lower New Sirocco is rising, and we need to act fast if we want to retain access to this part of the city, including the New Sirocco Mines. The player will need to identify the source of the rising magma level and stop it before it is too late.
Walkthrough

The Magma Level in New Sirocco is rising...the Settlement may be in danger within 150 days...Evangeline may have more information for you, after Building Tasks are done. The Settlement requires:

Speak to Evangeline and you will be informed about the rising magma level situation. Before we can continue the quest, we will once again need to complete some construction in New Sirocco. You will need a total of 4 specialized buildings, and have at least 2 upgraded buildings.

Once the construction is complete, speak to Evangeline again about the rising magma levels. It is decided that the rate at which the magma is rising is unnatural, and that some sort of magic must be the cause of it.
At this moment, a phantom of Chamberlord Orville appears in the City Hall and speaks to us, demanding that we return the Emberheart and leave our settlement immediately, or else face the wrath of the rising magma which he is commanding.

The Chamberlord is hidden at the end of the Smoking Crater Dungeon. It will become very apparent that it has opened. Make your way to the center of Caldera and enter the Scarlet Sanctuary.
Scarlet Sanctuary

Upon entering the Sanctuary, head west along the path and make your way past two Burning Men and a Ghost, until you enter a room with several doorways blocked by white light barriers.
First, head down the stairs and turn around to the east, where you can enter the only door not blocked by light barriers. Follow the path all the way to the east where you will find the first light lever and a Scarlet Emissary. Pull the lever and return to the room with the light barrier doors.
Back in the main room, you will find that the door leading to the south is now unblocked. Head along a short distance and you will immediately see another light lever, guarded by an Obsidian Elemental and several Burning Men. Pull the lever and return to the main room.
You will now find that the northern door in the main room is unlocked, enter it and follow the path to another Scarlet Emissary and the third light lever. Pull the lever and return to the main room.
The south-western door in the main room is now open, and after following the path you will eventually come to a room with two Obsidian Elementals and a Burning Man. Pull the lever here and return to the main room.
Finally, the north-western door in the main room is unlocked and you will come to an elevator. Take the elevator upwards, follow the path around through a blood wall and you will come to the Crimson Avatar waiting for you on a bridge.
Crimson Avatar
The fight against the Crimson Avatar is one of the most difficult in the entire game. It is advised to read the information on the Crimson Avatar article to be prepared for the fight ahead. With your best equipment and buffs prepared, enter the large red door to fight the avatar.
After defeating the Crimson Avatar, claim the Cracked Red Moon staff from the ground, and you will have a short conversation with the Scarlet Lady. She tells you that this is not the end, and she will be back for vengeance in the future.
Finishing Up

With the Cracked Red Moon, return to Evangeline in New Sirocco to complete the quest.
